Are you having any other trouble with inbound SMTP - instability, connectivity, DNS issues etc? This has happened to me on occasion and it's always when I'm having massive trouble with my SMTP servers. I'm configured with 2 smtp servers in 2 locations with 2 mx records and A records... When (God forbid) I'm having trouble with both servers for whatever reason (ISP issue, DNS issue, firewall issue etc), I get some duplicate messages during that period. I don't have a fix, it "goes away" shortly after we get SMTP services stablized. I chaulk it up to the sender's smtp server who continues to try to deliver the message and doesn't realize the message got through because the server went down prior to successfully accepting the message. I'm no smtp expert but it's not a chronic issue so I don't dig much deeper. Hope this helps.
